Patient Information

Financial Policy

We strive to deliver the finest care to our patients at the most reasonable cost. Our policy requires that payment is due at the time service is rendered unless other arrangements have been made in advance. For our patient's convenience, we accept payment by credit card, cash and check payments.

It is our policy to treat all patients and not to discriminate with regard to race, color, religion, national origin, age, sex, sexual orientation, gender identity or expression, or disability.

Please remember that patients are fully responsible for all fees charged by our office regardless of your insurance coverage. Most insurance companies will respond to claims submissions within four to six weeks. Any remaining balance after the portion that your insurance has paid is your responsibility. Your prompt remittance is appreciated.

If you have questions regarding your account, please do not hesitate to contact us.

Patient Forms

If you are a new patient to our office, please visit our Patient Forms page to download our new patient bundle with forms that will need to be filled out when you arrive at our office. Printing them, filling them out and bringing them with you will allow us to attend to your medical needs more quickly than completing them on your arrival.  Thank you and please contact our office if you have any questions at all.

Insurance Participation

Our practice makes every effort to provide our patients with the finest surgical care and the most convenient financial options. To accomplish this goal, we work to maximize our patient's insurance reimbursement for covered procedures. If you have any problems or questions, please ask our staff. They are well informed and up-to-date.

As a service to our patients, our practice accepts most dental insurance programs and we are participating providers for a variety of dental insurance plans. However, we remind our patients that the specific policy is an agreement between the patient and their insurance company. Please keep in mind that patients are responsible for the total obligation should your insurance benefits result in less coverage than anticipated. Our staff will gladly submit a pre-treatment estimate to your insurance company so that you will know what your benefits will be. Your policy may base its allowances on a fixed fee schedule, which may or may not coincide with our usual fee schedule. We advise patients that they should be aware that different insurance companies vary greatly in the types of coverage available.

It is important that patients bring their insurance (Medical & Dental) information to the consultation visit so that we can expedite reimbursement.

The following lists the dental insurances that we are participating providers:

1.) Aetna Dental

2.) Cigna Dental

3.)Guardian Dental

4.) Delta Dental
